This website has two forms. Both are optional, and you choose what to send.
Contact form: your name, email address, phone number (optional), the topic you select, and whatever you write in the message field.
Appointment request form: your name, email address, phone number (optional), the type of appointment you're interested in, your preferred day or time, and any notes you add.
We do not ask for, and you should not send, sensitive medical details, insurance ID numbers, Social Security numbers, or payment information through these forms. An appointment request is a request to be contacted — it is not a medical record and it is not a confirmed booking.
Our hosting provider records standard technical data such as IP address, browser type, pages visited, and referring page. This is used for security and to understand which pages are useful. We do not use advertising trackers or sell data to advertisers.
We do not sell, rent, or trade your personal information. We do not add you to a marketing list because you used a form — any newsletter signup is separate and always opt-in.

This website is not a patient portal. Clinical records created during an eye exam are maintained inside our practice management system under applicable healthcare privacy law, including HIPAA where it applies. Nothing you submit through this website becomes part of your clinical record automatically. Do not use these forms to discuss a medical emergency — call us, or in an emergency dial 911.
Form submissions live in our email system and are retained as long as needed to serve you and to keep reasonable business records. You can ask us to delete your enquiry at any time.
You may ask us to access, correct, or delete the information you submitted through this website. Email info@eyelandoptique.com or call 843 681 2020 and we'll take care of it. Depending on where you live, you may have additional rights under state privacy laws; we honor those requests regardless of residency.
This website is not directed at children under 13, and we do not knowingly collect their information online. Parents and guardians are welcome to request pediatric appointments on a child's behalf.
The site is served over HTTPS and form submissions are transmitted encrypted. No method of transmission over the internet is perfectly secure, so please don't send sensitive information through a web form.
